By signing below, I authorize JK Auto Body and Glass, LLC. to perform the requested repair work, including all necessary materials. I understand that JK Auto Body and Glass, LLC. may operate my vehicle for testing, inspection, or delivery at my own risk. I acknowledge that an express mechanic’s lien is placed on my vehicle to secure payment for all repairs and related charges. Full payment is due upon completion of the vehicle. I understand that JK Auto Body and Glass, LLC. is not responsible for loss of or damage to my vehicle or any personal items left inside due to fire, theft, accident, or any other causes beyond its control. I agree that storage charges will begin forty-eight (48) hours after repairs are completed. I understand that my vehicle may be considered abandoned if it remains at the facility for more than fifteen (15) days after delivery without authorization to diagnose reported issues, more than ten (10) days after diagnosis is completed without authorization to proceed with repairs, or more than ten (10) days after repairs are completed and I have been notified. If my vehicle is deemed abandoned, I authorize JK Auto Body and Glass, LLC. to have the vehicle removed from the property at my expense. I agree to hold JK Auto Body and Glass, LLC. harmless from any costs associated with this action, including towing, storage, and related fees. I accept full responsibility for any damage that may occur during removal or while the vehicle remains on the premises during the abandonment period. I further acknowledge that once a vehicle is deemed abandoned, JK Auto Body and Glass, LLC. is not responsible for the safekeeping, condition, or security of the vehicle in any way. I agree that if legal action is required to enforce this agreement, I will be responsible for all reasonable attorney’s fees and court costs.
